(AP) ? A deadly flare-up in fighting between Israel and Gaza's militant Hamas group subsided on Thursday after Egypt helped to restore calm ahead of a major Muslim holiday.

Weeks of simmering violence had intensified on Tuesday night, when rocket fire from Gaza drew Israeli airstrikes that killed two Palestinian militants. The hostilities came to a boil Wednesday, when militants fired some 80 rockets and mortars at southern Israel, and Israeli aircraft struck Gaza four times. In all, four Palestinians, including three militants, were killed in the fighting and two foreign workers in Israel were critically wounded.

The rocket and mortar fire stopped altogether overnight, though one projectile landed in southern Israel on Thursday morning, causing no damage. The military said it last struck Gaza on Wednesday morning. The violence ebbed as Muslims began preparing for the Eid al-Adha holiday, which begins on Friday.

Both sides confirmed Egyptian involvement in ending the fighting.

Under longtime leader Hosni Mubarak, Egypt had played an important role in halting multiple outbreaks of hostilities between Israel and Gaza militants. The new government of Egyptian President Mohammed Morsi who belongs to Hamas' parent movement, the Muslim Brotherhood, kept up the tradition.

Israeli defense official Amos Gilad told Army Radio on Thursday that Egyptian security forces have "a very impressive ability" to convey to the militants that it is in their "supreme interest not to attack."

Hamas spokesman Ayman Taha said Egypt conveyed Israel's desire to contain the violence.

"We said we'll abide by the calm if the occupation abides," he said. "It happened over the phone with Egyptian intelligence."

Hamas, which has killed hundreds of Israelis in suicide bombings and other attacks, has largely avoided attacks since a devastating Israeli military offensive nearly four years ago. It remains virulently anti-Israel but has sought to keep things quiet as it consolidates control of Gaza, which it violently overran five years ago.

Still, it is under pressure from smaller groups to prove that it remains in confrontation with the Jewish state.

Israeli Foreign Minister Avigdor Lieberman suggested the visit Tuesday of Qatari's leader to Gaza ? the first by a head of state to the territory since it came under Hamas rule ? emboldened Hamas to clash this week with Israel by bolstering the group's legitimacy.

Tampa, Florida, 23 October 2012Top marine predators like tuna and sharks are suffering from the effects of climate change as the availability of prey decreases and the spatial distribution of their prey shifts. Countless other marine plants and animals are also affected.

One way to adapt to or mitigate these changes is to design marine protected areas (MPA) and MPA networks that integrate these and other climate-related considerations. Accordingly, the Commission for Environmental Cooperation (CEC) has published Scientific Guidelines for Designing Resilient Marine Protected Area Networks in a Changing Climate in collaboration with the International Council for the Exploration of the Sea (ICES) and based on the work of thirty-three of North America's top experts. The published guidelines were launched today at the Restore America's Estuaries Conference in Tampa, Florida. To download the guidelines, click here.

Climate change is affecting the earth's oceans and many of the species that depend on them. Warmer ocean temperatures are being associated with smaller populations of phytoplankton and zooplankton, an important food source for fish and marine mammals. Rising coastal sea levels may impact nesting sites for turtles and habitat for marine birds. Also, the carbon cycle is being impacted by warmer temperatures and ocean acidification. This bleaches and kills coral reefs, a major undersea habitat and nursery for countless species of fish, shellfish and crustaceans of vital importance to us, and is negatively influencing natural carbon sinks such as mangroves, salt marches, seagrasses, and tidal wetlands, reducing their ability to remove carbon dioxide from the atmosphere.

MPA size, placement, and their respective role in reducing pressures such as fishing and coastal habitat conversion, are just some of the considerations for designing resilient MPAs in light of climate change. The CEC's practical set of guidelines will help scientists, MPA planners and managers improve their ability to design, connect, manage, assess and adapt MPAs and MPA networks to potential climate change at national and continental scales. The guidelines are broken down into four sections:

1. Protect species and habitats with crucial ecosystem roles, or those of special conservation concern

2. Protect potential carbon sinks

3. Protect ecological linkages and connectivity pathways for a wide range of species

4. Protect the full range of biodiversity present in the target biogeographic area

In November 2012 the CEC will be publishing a companion piecea practical guide for MPA managers and network planners on how to implement these guidelines. North America is an ideal region to pilot this global effort because of the interconnectivity of its oceans, diverse marine life and its nearly 2000 MPAs. The guidelines identify some of the benefits of working collaboratively across a region such as North America, considering that connectivity is particularly important for migratory species and species that move throughout their life stages. The guidelines also advocate that international collaboration strengthens capacity through shared experiences.

Marine protected area experts and officials in Canada, Mexico and the United States have been working together over the past decade through the CEC's North American Marine Protected Areas Network (NAMPAN) to conserve North America's marine biodiversity. The guidelines were developed by Robert J. Brock (NOAA MPA Center), Ellen Kenchington (Fisheries and Oceans Canada) and Mara Amparo Martnez-Arroyo (Universidad Nacional Autnoma de Mxico), in collaboration with the International Council for the Exploration of the Sea Study Group on Designing Marine Protected Area Networks in a Changing Climate (ICES-SGMPAN). They realize that other people should be free to live their own lives, you know, should be free to get married to who they want because at the end of the day, they?re allowed to get married to whoever they want.? American Academy of Pediatrics weighs in for first time on organic foods for children

ScienceDaily (Oct. 22, 2012) ? Parents know it's important for children to eat a wide variety of fruits and vegetables, low-fat dairy products, and whole grains. But it's less clear whether spending the extra money on organic foods will bring a significant benefit to their children's health.

To offer guidance to parents -- and the pediatricians caring for their children's health -- the American Academy of Pediatrics (AAP) has conducted an extensive analysis of scientific evidence surrounding organic produce, dairy products and meat. The conclusion is mixed: While organic foods have the same vitamins, minerals, antioxidants, proteins, lipids and other nutrients as conventional foods, they also have lower pesticide levels, which may be significant for children. Organically raised animals are also less likely to be contaminated with drug-resistant bacteria because organic farming rules prohibit the non-therapeutic use of antibiotics.

However, in the long term, there is currently no direct evidence that consuming an organic diet leads to improved health or lower risk of disease. No large studies in humans have been performed that specifically address this issue.

"What's most important is that children eat a healthy diet rich in fruits, vegetables, whole grains, and low-fat or fat-free dairy products, whether those are conventional or organic foods. This type of diet has proven health benefits," said Janet Silverstein, MD, FAAP, a member of the AAP Committee on Nutrition and one of the lead authors of the report. "Many families have a limited food budget, and we do not want families to choose to consume smaller amounts of more expensive organic foods and thus reduce their overall intake of healthy foods like produce."

The AAP report, "Organic Foods: Health and Environmental Advantages and Disadvantages," was released on Oct. 22 at the AAP National Conference & Exhibition in New Orleans. It will be published in the November 2012 issue of Pediatrics (published online Oct. 22). The report outlines the research that has been conducted on organic foods, including convincing evidence of lower exposure to pesticides and less contamination of livestock with drug-resistant bacteria.

"At this point, we simply do not have the scientific evidence to know whether the difference in pesticide levels will impact a person's health over a lifetime, though we do know that children -- especially young children whose brains are developing -- are uniquely vulnerable to chemical exposures," said Joel Forman, MD, FAAP, a member of the AAP Council on Environmental Health and one of the lead authors of the AAP clinical report.

If cost is a factor, families can be selective in choosing organic foods, Dr. Forman said. Some conventionally grown fruits and vegetables tend to have lower pesticide residues. The AAP cites organic shopper's guides like those provided by Consumer Reports and the Environmental Working Group as references for consumers. The AAP found no individual health benefit from purchasing organic milk, but emphasizes that all milk should be pasteurized to reduce the risk of bacterial infections. Raw milk increases the risk of serious infection with bacteria including Salmonella, E. coli, Listeria, Campylobacter and Brucella.

Purchasing meat from organic farms that do not use antibiotics for nontherapeutic uses has the potential to reduce antibiotic resistance in bacteria that infect people. The AAP calls for large, well-designed, prospective cohort studies that directly measure environmental exposures such as estrogen at low levels to understand the impact of hormonal exposure of children through milk and meat.

The AAP report also notes that the motivation to choose organic produce, meat and dairy products may be reasonably based on larger environmental issues, as well as human health impacts like pollution and global climate change.

"Pediatricians want families to have the information they need to make wise food choices," said Dr. Forman. "We hope that additional research will improve our understanding of these issues, including large studies that measure environmental exposures and neurodevelopment."

Mass. gov.: Fungus seen at meningitis-linked firm

BOSTON (AP) ? State officials investigating a pharmacy linked to a deadly outbreak of meningitis said Tuesday they found shoddy sterilization practices and unclean conditions there, including debris-covered floor mats and standing water from a leaking boiler.

State officials also said the New England Compounding Center shipped steroids from the possibly contaminated batches suspected in the outbreak before it received its own test results confirming the drugs were sterile.

Gov. Deval Patrick said he's ordered state pharmacy regulators to conduct surprise inspections ? the first of which happened Tuesday ? at companies similar to the NECC and take other steps to tighten oversight. The state also has moved to revoke the company's operating license and the licenses of its top three pharmacists.

"Those whose laboratory practices caused this outbreak should never practice pharmacy or manufacture in Massachusetts again," Patrick said.

The outbreak of fungal meningitis, an inflammation of the lining of the brain and spinal cord, has sickened 308 people, including 23 who have died, in 17 states. The outbreak has been linked to a steroid made by the NECC and taken mainly for back pain. Compounding pharmacies like NECC custom mix solutions in doses or forms generally not commercially available.

The federal government is conducting a criminal investigation.

The state said Tuesday that its preliminary investigation, which began last month after the company was first suspected in the growing outbreak, found large batches of drugs ready for general distribution but not labeled for specific patients.

Its state license permits the company to fill out only specific prescriptions for specific patients, and distributing drugs in bulk like a manufacturer would violate that, said Dr. Madeleine Biondolillo, director of the state Department of Public Health's Bureau of Healthcare Safety.

But company attorney Paul Cirel said it's "hard to imagine" state regulators weren't previously aware of the scale of its operations because they've worked so closely together. The state Board of Pharmacy has always had complete access to the facility, and board members were there as recently as last summer, he said.

"NECC's transparency in dealing with the board since inception in 1998 demonstrates its good-faith intention to operate in compliance with the requirements of its license," Cirel said.

Besides possible state license violations, Biondolillo said the inspections also revealed "several health and safety deficiencies" at the NECC facility in Framingham, just west of Boston.

Three lots of steroids produced by the company are suspected in the outbreak, and the company shipped orders from those lots 13 times before receiving the results of its own tests to confirm those lots were sterile, Biondolillo said. Some medication was shipped as many as 11 days before the company received test results, she said.

Biondolillo also detailed signs of flawed sterilization procedures, including black specks of fungus in sealed vials of the steroids, which were returned to the company during a recall.

Investigators found the company didn't sterilize its products long enough and didn't adequately test whether its sterilization equipment was working, she said.

In addition, mats on which people wiped their shoes to remove contamination before entering a sterile environment were "visibly dirty and soiled with assorted debris," she said. And a leaking boiler adjacent to a pharmacy clean room left an unsanitary pool of water around it and the adjacent walls, she said.

None of what's been found is enough to definitively determine what caused the contamination, and the investigation is ongoing, Biondolillo said.

Meanwhile, Patrick's moves to increase oversight at the state's 25 compounding pharmacies have already started.

The first of the unannounced inspections, to take place at least annually, was done on Tuesday, health department spokesman Alec Loftus said. He wouldn't give the inspected facility's name and said the results are being reviewed.

Patrick said compounding pharmacies will now be required to file annual compliance reports that could help regulators determine if they are acting as manufacturers.

Insect bites ? Medical Webbook

Tiny puncture wounds in the skin inflicted by blood. sucking insects, such as osquitoes, lice, midges, gnats, horseflies, sand flies, fleas, and bedbugs. Some small arachnids (eight-legged animals similar to insects), such as ticks and mites, can?cause similar injuries.

Most bites cause only temporary pain (or itching for a day or two) although some people have severe skin reactions. In the tropics and subtropics, insect bites are potentially more serious because certain biting species can transmit disease (see Insects and disease).

Causes

Insects that bite do so to obtain a blood meal. The mouthparts of biting insects are specially adapted for piercing skin and sucking blood. Insect bites are most common on exposed parts of the head, hands, arms, or legs. Although mosquitoes (which attack mainly after dark) may be the most troublesome biting insects, many bites blamed on mosquitoes are in fact caused by cat or dog fleas. These fleas inhabit various domestic locations where the pet habitually rests (e.g., carpets, sleeping baskets, or sofas); when their normal host is absent, they may jump onto humans to feed.

Of the more easily visible insects, horseflies can produce a particularly painful bite, while gnats can be a menace if encountered in a swarm.

Symptoms

All insect bites provoke a reaction in the skin that is primarily an allergic response to substances in the insect?s saliva or its feces, which are often deposited at or near the site of the bite and rubbed in by scratching. Reactions vary from innocuous red pimples to painful swellings (which may weep) or an intensely itching rash. People vary in their reactions to the same biting insect; in some people the reaction is extremely severe.

Avoidance

Avoiding insect bites can be particularly important for campers and hikers, anyone living in a mosquito infested area, and travelers or residents in tropical countries.?Bites outdoors can be reduced by wearing trousers, socks, and long sleeved?shirts (especially after dark, when mosquitoes are most active), and by using insect repellents. Indoors, bites can be reduced by using insect screens over open windows and by spraying bedrooms with aerosols containing pyrethroid insecticides?before going to bed. If this fails, it may be necessary to use mosquito nets and slow burning antimosquito coils that give off pyrethroids.

Treatment

Bites or bitten areas should be thoroughly washed with soap and water, and a soothing ointment, such as calamine lotion, should be applied. Scratching should be avoided. If there is a severe reaction, a physician should be called; a cream containing an antihistamine may be required.?Severe itching on the scalp or in the pubic hair suggests the possibility of a louse infestation, which is treated with insecticidal lotions (see Lice). In the case of flea bites, the entire residence (not just the pet) may require treatment with insecticide to Kill the flea population. (See also Spider bites; Mites and disease; Ticks and disease.)

ESPN?s ETHAN SHERWOOD Strauss has?a remarkable story from Game 7 of the 1993 Rockets-Sonics series: Houston had the ball and were down 101-100 with 13 seconds left when reserve player Winston Garland walked onto the court during the play and became his team?s sixth man.

That?s right, a player jumped off the bench, went onto the court, and tried to illegally play during the final seconds of Game 7 of an NBA playoff game.

No one caught it at the time, but then-Sonics coach George Karl told Sherwood Strauss?they were dumbfounded when they were watching film a few days later and noticed it. Garland himself still refuses to acknowledge that any cheating took place, despite this video.

The play ultimately came of nothing, and the Sonics won the game and the series. But still, it?s amazing that this blatant cheating attempt didn?t go public until now.

Here?s the video. Keep your eye on the Rockets bench. Garland is kneeling on the sidelines before he gets up, takes the court, and gets in a rebounding position when the shot goes up:

Another remarkable tidbit: It was actually sound strategy because of a loophole in the rules.

If Garland got caught, they would have given him a technical but they couldn?t take away any points that he scored, according to ESPN. So if he hit a three to put the Rockets up 103-101, it would have counted.
